CPSA Number (Licence for Independent Practice)

The College of Physicians and Surgeons of Alberta (CPSA) grants medical licences needed for independent practice.

  • Deadline to apply is April 1st, for a July 1st start
  • For a new licence, complete the application for medical registration. Once your Review of Qualifications is received from PhysiciansApply, follow instructions in the Review of Qualification Results communication (Overview of process here)
  • Fees: Review of qualifications - $200; Registration - $800; Practice Permit Fee $1,960

Hospital Privileges

Most applicants are recruited for Appointment through zone-specific processes that may vary by zone or by site. To apply for Appointment:

  • Applicants may be asked to complete the Request for Application Package form and submit it to the appropriate Zone Medical Affairs Office (ZMAO). The Application Package is then sent to the applicant for completion and submission to the ZMAO.
  • The Application Package may be supplied by the ZMAO as part of the recruitment process and would not require the Applicant to fill out the “Request for Application Package” form.

  • Generally, copies of the following documents are required:
    • Resume
    • CMPA proof of insurance
    • Medical licence
    • Three professional letters of reference
